Aim

The Clinical Atlas is designed to improve the quality of diagnosis and clinical management of patients with thyroid tumors.

Practical usefulness (advantages and benefits)

Studying the Atlas material will allow you:

  • to be aware of all the basic ultrasonic types of benign and malignant thyroid nodules, which is important in considering the need for FNA performance;
  • to refer patients for primary or repeated aspiration biopsy of nodules only in those cases where this  is really necessary and, at the same time, not puncture all the nodules indiscriminately, which results in a considerable number of non-informative, uncertain, even erroneous results in some cases;
  • provide a more accurate clinical assessment of the results of biopsy, which has certain limitations, a constant level of false positive results, and a different diagnostic accuracy for different types of nodules;
  • provide a more accurate clinical assessment of the combined ultrasound and FNA results, which will allow to avoid unnecessary surgical interventions in a considerable part of patients with benign nodules, and timely reveal the malignant process that needs surgical treatment.

Source and reliability of material

The results are based on author’s clinical experience and long-term research studies of ultrasound, cytological, and pathomorphological features of thyroid tumors, carried out at the State Institution “V.P. Komisarenko Institute of Endocrinology and Metabolism, National Academy of Medical Sciences of Ukraine”. The diagnostic accuracy of the data obtained has been verified by over 15 years of real clinical practice, being confirmed on thousands of patients. The results of the research are unique, having no analogues in the literature.

Content, methods, and scientific novelty

The book presents a comprehensive diagnostic phase of the Clinical protocol for the management of patients with benign and malignant thyroid tumors: from detailed ultrasound and cytological (FNA) diagnoses to current clinical guidelines - based on US and FNA results - for further patients’ management.

The following issues are considered in detail:

  • seven major individual ultrasonic features of nodules: shape, border, echogenicity, cystic cavities, hyperechoic structures, vascular structure, enlargement of regional lymph nodes, and their morphological basis;
  • 10 combined ultrasonic models and their pathomorphological correspondence to specific types of benign and malignant thyroid tumors (Thyroid Nodules Diagnostic Ultrasound Models – ThyrNoDUM Classification, S. Matiashchuk, 2004);
  • results of FNA for each ultrasonic model in operated patients (cytologic results according to AACE/AME/ETA Thyroid Nodule guidelines, 2010);
  • indications for aspiration biopsy for each ultrasonic model;
  • indications for surgical treatment for each ultrasonic model taking into account FNA results, in accordance with the modern international Clinical protoco(AACE/AME/ETA Thyroid Nodule guidelines, 2010).

The book includes a large Appendix that comprises real clinical cases of diagnosis and management of patients with various types of benign and malignant thyroid tumors.

The Atlas is widely illustrated, contains 575 pictures which greatly improve the understanding of the material.
